1) First and Last Name
2) Date of Birth (DOB)
3) Medical Conditions
4) Life Threatening Medications (i.e. blood thinners)
5) Allergic to: _____________
6) Emergency Contacts (ICE: name, phone)
When placing your order our Engraving Preview tool will show you how your engraved message will appear on your medical jewelry.
Engraving your name on your medical ID is a personal preference but we recommend that at least your first name is included. In an emergency, emergency responders will say a person’s name to get their attention and check for responsiveness. We recommend that all Alzheimer’s, Dementia and other mentally compromised patients engrave their name and address or contact information of a friend, caregiver or family member.
List only those medical conditions that would be important in an emergency. For example, a minor surgery that took place several years ago may no longer be relevant to your current medical care.
List only medicines you wish to be engraved. Generally, this would be prescription medicine, taken on a long-term daily basis which are unlikely to change. You may wish to list the most important medicines first.
List only those allergies which are severe and can cause a life threatening event.
Space is limited, so summarize your information with short, descriptive words. We provide Emergency Medical Cards with every purchase that can include as much detail as you would like.

I’m a 46 year old female and I purchased this bracelet to replace my old one after being diagnosed with yet another condition that needed to be listed. My last bracelet was sterling silver and was a pain to get on and off by myself and the clasp was starting to catch on things. This one is much easier to handle by myself. I like that there is plenty of character space for my conditions and my ICE info as well. Definitely does not look masculine on me but I’m on the tall end at 5’7″/150#. Very happy with purchase.

I have allergies to foods and medications and I needed a medical id bracelet that would be able to show all of this information, since omitting anything would not be an option. What I like about this bracelet is that the area on it is big enough to hold all of my medical info. The engraving is nice and clear, making it easily readable which is very important. I also like the black leather adjustable band. It fits nice around my wrist and it would be easy to spot by medical personnel.

High quality bracelet. Leather is good quality and the metal ID and the medical symbol slide around so you can put them where ever it’s comfortable for you. Printing on the bracelet seems to be good quality as well. For the price I’m very pleased with it.

Absolutely Perfect…and the cool thing about it is that you can actually CUT the end of the leather strap to adjust it if you want.I didn’t try to cut it absolutely perfect because I feared that I would end up with no strap if I tried to make it absolutely perfect. I cut it to where it looks good enough and besides no one will be looking at the end of my strap with a magnifying glass.
