1) First and Last Name
2) Date of Birth (DOB)
3) Medical Conditions
4) Life Threatening Medications (i.e. blood thinners)
5) Allergic to: _____________
6) Emergency Contacts (ICE: name, phone)

Engraving your name on your medical ID is a personal preference but we recommend that at least your first name is included. In an emergency, emergency responders will say a person’s name to get their attention and check for responsiveness. We recommend that all Alzheimer’s, Dementia and other mentally compromised patients engrave their name and address or contact information of a friend, caregiver or family member.
List only those medical conditions that would be important in an emergency. For example, a minor surgery that took place several years ago may no longer be relevant to your current medical care.
List only medicines you wish to be engraved. Generally, this would be prescription medicine, taken on a long-term daily basis which are unlikely to change. You may wish to list the most important medicines first.
List only those allergies which are severe and can cause a life threatening event.
Space is limited, so summarize your information with short, descriptive words. We provide Emergency Medical Cards with every purchase that can include as much detail as you would like.

VERY sturdy — and that pleases me greatly. The engraving and symbol are permanent, and I really like that because I’ve had others that didn’t last beyond a few wearings. The price is excellent. I would like to see something a bit more feminine that is still this sturdy, but this is a necessary item, so I will wear this, even though it is very big in scale for a woman. I’m not sure that’s possible with the symbol and the amount of engraving required.

This condition has been very overwhelming for me and my family. I am still in my thirties, and have never really been sick before this. I was recently implanted with a defibrillator. If you look at me you would never know that I have one. This bracelet brings me comfort, if I am unable to speak people will know.

The bracelet is great, nice weight to it and exactly as described. Our only problem is that we cannot register my husband’s info online since we don’t have the required product code and can’t find anyway to get it. If we had bought it directly from theamufacturer the registration would be simpler but it’s not a big deal. The bracelet does it’s job which is to alert that he has a defibrillator pacemaker.

This is a bit on the clunky side however, it is heavy and made well. It isn’t made to be pretty. There are others that would work for a special occasion but I got this for everyday wear with durability in mind. The label part slides around to always be on the underside of your wrist because of the weight so be prepared to have the fastening clip on top most of the time.

Great bracelet.Very clear,very study.I looks mostly like a men’s bracelet,but of you want sturdy and practical, this could work for women.Unless you are very delicate with tiny wrists. I ordered it for my son,he lobed it and wears is day and night.It is holding up well.
