Any one of these websites may ask for and collect personally identifiable information in order to enhance your experience and provide you with relevant information. This is the online privacy policy (“Policy”) of Universal Medical Data, LLC and its parent, affiliate and subsidiary entities (collectively “The Company”). This Policy applies only to activities The Company engages in on its websites (the “Sites”) and does not apply to The Company’s activities that are “offline” or unrelated to the Sites. To better protect your privacy we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used.

We generally keep this Privacy Policy posted on our Website and you should review it frequently, as it may change from time to time without notice. The Information We Collect:

Non-Personal Information: When you visit our Sites, we track certain information that is non-personally identifiable. We may make message boards, chat rooms, and other interactive forums available as part of the services offered by the Company. You should be aware that any information which you post to these interactive forums or otherwise choose to make publicly available, including your Personal Information, may be disclosed and available to all users who have access to that portion of the Site or Services, and is therefore no longer private. IP Addresses: An IP address is a number that is automatically assigned to your computer whenever you are surfing the Internet. We collect IP addresses for purposes of system administration, to report non-personal aggregate information to others, and to track the use of our Website. The Company is not responsible for the privacy policies of websites to which it links. If you provide any information to such third parties different rules regarding the collection and use of your personal information may apply. We strongly suggest you review such third party’s privacy policies before providing any data to them. We are not responsible for the policies or practices of third parties. Please be aware that our sites may contain links to other sites on the Internet that are owned and operated by third parties. The information practices of those Web sites linked to our Sites is not covered by this Policy.
