The Access PHR PLUS provides this access with the health information you choose in an organized manner.

Your Personal Health Record can hold the details of your complete medical history. This information can be accessed by a medical responder from any internet capable device by entering your unique member ID number.

The Access PHR PLUS includes your choice of a hypoallergenic, waterproof silicone medical alert bracelet or, for those wishing to be more discrete, a medical alert keychain.  Either choice includes an acrylic plate that fits neatly in the pocket. This plate is engraved with clear instructions for accessing your medical information.

Many medical emergencies require immediate triage to stabilize you before you can be transported to a medical facility. Being able to access your most life-threatening medical conditions or life-saving medications you may need; is the reason we have integrated a formatted emergency medical ID wallet card with the information in your PHR that you can print out and carry with you.

The Elite II medical alert bracelet has the Star of Life medical alert symbol on the front face.  This is subtle yet easily recognized worldwide to make it known of an existing medical condition.  The instructions, SEE BACK FOR MED INFO, surround the symbol on the face of the bracelet for added assurance that, while your information is tucked away and out of view, it is not overlooked.  The engraved acrylic plate with your PHR access instructions and secure member ID and is safely stored in the bracelet pocket on the underside of the bracelet.
