1) First and Last Name
2) Date of Birth (DOB)
3) Medical Conditions
4) Life Threatening Medications (i.e. blood thinners)
5) Allergic to: _____________
6) Emergency Contacts (ICE: name, phone)

Engraving your name on your medical ID is a personal preference but we recommend that at least your first name is included. In an emergency, emergency responders will say a person’s name to get their attention and check for responsiveness. We recommend that all Alzheimer’s, Dementia and other mentally compromised patients engrave their name and address or contact information of a friend, caregiver or family member.
List only those medical conditions that would be important in an emergency. For example, a minor surgery that took place several years ago may no longer be relevant to your current medical care.
List only medicines you wish to be engraved. Generally, this would be prescription medicine, taken on a long-term daily basis which are unlikely to change. You may wish to list the most important medicines first.
List only those allergies which are severe and can cause a life threatening event.
Space is limited, so summarize your information with short, descriptive words. We provide Emergency Medical Cards with every purchase that can include as much detail as you would like.

I had an older tag from this company and this one was a bit thinner but much wider to allow more info to be engraved on it. This one has 18 lines w at least 20 characters a line. I am happy about the purchase and again got the basic black. I was happy that it was delivered quickly and that it had its own chain.
My only complaint is that one line my wife’s name was out of order from the way I typed everything in. That might lead to minor confusion if a responder was trying to call her and instead called my parents but all names and numbers were spelled correctly so I am still satisfied. They did deliver my tags a month before the date promised.

The actual tag is well made and is easy to read. The draw back is the chain. Now I’m a good size man and the chain barely fits over my head. Once in place it’s great. Now if the chain was more than an ordinary light pull chain I would have given it five stars. If it were more substantial and had a clasp that would allow for easy and repeated open and closing it would be great. I will be changing them chain.

I got this as a gift for my father. There was very quick delivery arrived about a week earlier then expected. The engraving is clean and easy to read it is better then expected for the price. I had made some spelling errors in some medication my father was allergic to and the company noticed it and called to inform me of it before the engraving. I had an older stainless steel style medical alert dog tag with personal information that was out of date. I looked at a variety of different options and chose this blue aluminum tag. I was expecting to have inset lettering like my old tag, but this is obviously laser or chemical etched. The result is a smooth surface with printing available on both sides of the tag. I was worried that the coloring would degrade and the lettering be difficult to read as it aged, but so far so good. I liked the ability to have information on both sides of the tag with plenty of lines available for medication and contact information. I choose to put medication information on one side and contact information on the other. The number of lines of information and number of characters per line made getting what I wanted on the tag easy. Others have noted that Universal Medical Data provided very personalized help to determine what information to incorporate. I found that I received an e-mail (almost immediately) that offered help via links to FAQs. Not the level of detail perhaps that other reviewers indicated, but completely sufficient for me.
Having the tag made out of aluminum has several advantages over my old stainless steel tag. First, for a tag at least 50% larger in area it is much lighter (and thinner). Second, aluminum warms quicker that steel, so putting it on you don’t have that sensation of something cold against your chest for very long. And finally, it weighs a LOT less than the steel tag, so that I hardly know it’s there. Overall, I really like this tag!
