1) First and Last Name
2) Date of Birth (DOB)
3) Medical Conditions
4) Life Threatening Medications (i.e. blood thinners)
5) Allergic to: _____________
6) Emergency Contacts (ICE: name, phone)

Engraving your name on your medical ID is a personal preference but we recommend that at least your first name is included. In an emergency, emergency responders will say a person’s name to get their attention and check for responsiveness. We recommend that all Alzheimer’s, Dementia and other mentally compromised patients engrave their name and address or contact information of a friend, caregiver or family member.
List only those medical conditions that would be important in an emergency. For example, a minor surgery that took place several years ago may no longer be relevant to your current medical care.
List only medicines you wish to be engraved. Generally, this would be prescription medicine, taken on a long-term daily basis which are unlikely to change. You may wish to list the most important medicines first.
List only those allergies which are severe and can cause a life threatening event.
Space is limited, so summarize your information with short, descriptive words. We provide Emergency Medical Cards with every purchase that can include as much detail as you would like.

I love the look. Easy to clean. My child is 6 and tends to be finicky with things. He had been taking it off during school and bending it back and forth. After only a couple days, it started to crack. I was hoping it would hold up better. Hard to find something for kids that lasts and that won’t bother them for medical alert.

I like the look of it, but when trying to size it I had great difficulty. First, I saw no lines for cutting, then when trying to reassemble it, I couldn’t get the latch closed. While trying to do this, the other end came undone. I thought I had both ends closed but the end I had initially opened to size came undone. I sent it back to the company to fix, but if I continue to have problems will be asking for a refund.

I did not like the clasp for this. It was extemely hard to set up and you have to cut off the extra. My son is very skinny from just getting diagnosed with Type 1 diabetes so his wrists are thin now but not for long. Now we can’t make it bigger.

The bracelet is great however I thought by looking at the picture and reading the description that is was 2 separate bracelets and not one made up of two colors.. that’s the only complaint honestly I believe it needs to be added into the description that it is 1 bracelet made of two colors ..

I love that this medic alert ID has room for so much information, and even has room on the back for more. However, the medic alert insignia is not very well defined – it doesn’t stand out. In addition, the insignia is in a place that isn’t readily visible, and that’s the whole purpose of having it on the bracelet – so folks know what it is. I have already thrown the bracelet away. But I kept the metal part, and added it to another medical alert bracelet that works better for me.
