1) First and Last Name
2) Date of Birth (DOB)
3) Medical Conditions
4) Life Threatening Medications (i.e. blood thinners)
5) Allergic to: _____________
6) Emergency Contacts (ICE: name, phone)

Engraving your name on your medical ID is a personal preference but we recommend that at least your first name is included. In an emergency, emergency responders will say a person’s name to get their attention and check for responsiveness. We recommend that all Alzheimer’s, Dementia and other mentally compromised patients engrave their name and address or contact information of a friend, caregiver or family member.
List only those medical conditions that would be important in an emergency. For example, a minor surgery that took place several years ago may no longer be relevant to your current medical care.
List only medicines you wish to be engraved. Generally, this would be prescription medicine, taken on a long-term daily basis which are unlikely to change. You may wish to list the most important medicines first.
List only those allergies which are severe and can cause a life threatening event.
Space is limited, so summarize your information with short, descriptive words. We provide Emergency Medical Cards with every purchase that can include as much detail as you would like.

This is a good bracelet, my only problem with it is I have some skin issues (eczema being one of them). When this bracelet gets whet my skin gets damp underneath causing a clear up of my eczema. Make sure you measure your wrist for an accurate fit because you do have to cut and adjust the bracelet to fit yourself.

I don’t really like anything on my wrist so any bracelet is a stretch. It’s a good idea to have a medical indication when you’re not with family or friends who know about any condition. I got this when I was traveling, just in case. It’s discrete enough.

I have had this bracelet for a couple of months now. I needed something that would hold more information than most, and I wanted something that would last longer than the stainless one I had purchased previously. This one holds plenty of information and the black print makes it much easier to read than my previous one that was just engraved. So far the print has stayed on, but there are some scratches already on the front that cut through the print so I’m not sure how much of a problem that will be. I have been buying these bracelets for years and I find I pretty much have to replace them yearly regardless of what I spend on them so I tend toward the less expensive option (I have purchased a couple for over $100 and they haven’t lasted any longer than the $20 ones). I’m not sure the print on the front of this one will last that long, but I keep the important info on the back and this one is MUCH easier to read than the ones that are just engraved. The band is fine; I like the red because it’s more noticeable than some of the decorative ones I’ve worn (which ER staff have overlooked as just jewelry). Communication with the company was easy, I received the bracelet quickly, it was easy to size, and I can actually just slip it on and off when I shower rather than having to use the clasp. So far it’s one of the better bracelets I’ve purchased, and the price was great. I would recommend this to others, assuming you have realistic expectations for how long the print on any of these will last.

we bought this for our 9 year old, he has autism, so we wanted something in case anything happened with his basic info and contact numbers. It is much better than any other autism bracelets that we have purchased. this one has all the information engraved, so it doesn’t come off. We did buy some additional “badges” that had the autism logo, just to make sure there was something that people could see right away. I liked it so much i bought one for my mom as well. I recommend this item.

I bought this after my doctor advised me to wear one. I love that I can have 9 lines of information, I have not seen any other products allow that much room. My only complaint is the medical symbol peeled off the front side. I only take this off to shower so it’s well worn. I’ve had it for close to a year and the plate is a little scratched but still very legible. My only advise to the maker would be to put an extra small plate with the medical sign that a lot of the other brackets have, or allow room for it on the plate but keep the 9 lines!! Well worth the 34.00, for the peace of mind it provides.

Great medical alert band. The engraving is very clear and love it’s on both sides. Easy to trim and get the right fit. My only complaint would be that the medical symbol needs to be closer to the top where the engraved plate is, not at the clasp where it’s currently located.

The ability to get 9 lines of information on the bracelet Is what sold me on the bracelet. A feature I would like to see added is the ability of the bracelet to be scanned by smart phone to immediate online information. The online information can easily be updated.
